For your convenience we accept Visa, MasterCard and Debit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your benefit coverage.

As a courtesy to you, we will bill your primary benefit provider and track claims. Please keep us informed of any changes to your benefit plan. We are able to provide you with a dental claim form which you may submit to your secondary benefit provider if applicable. You are responsible for the fees charged by our office, no matter what your plan coverage may be. Any remaining balance is your responsibility.
